Illumination is not the same as visibility
A 100% illuminated moon behind an overcast deck is invisible; a 12% crescent on a clear, dry night is sharp and obvious. Visibility depends on the phase, the moon's altitude above the horizon during the hours you are actually outside, the cloud cover during those specific hours, and how much humidity and haze sit in the lower atmosphere. Each destination page evaluates all of them together instead of quoting a phase in isolation.
Why the moon is sometimes missing from the night sky
Around new moon the moon rises and sets with the sun, so it is above the horizon only in daylight. Even a few days later, a crescent may set within an hour of sunset. This is the most common reason people conclude the forecast was wrong: the sky was clear, but the moon had already gone below the horizon.
